What Is Metabolic Syndrome?

Metabolic syndrome is a cluster of interconnected risk factors — visceral fat accumulation combined with high blood pressure, elevated blood sugar, and lipid abnormalities. Individually, each factor may be borderline; combined, they dramatically increase the risk of cardiovascular disease, stroke, and type 2 diabetes.

In Japan, an estimated 1 in 2 adult men and 1 in 5 adult women meet criteria for metabolic syndrome or its precursor state.

Japanese diagnostic criteria require the waist circumference threshold (visceral obesity) PLUS 2 or more of three risk factors.

Waist circumference is a proxy for visceral fat accumulation, measured at the navel level.

Why is the women's threshold higher? Because a visceral fat area of 100cm² corresponds to approximately 85cm waist in men and 90cm in women. Women tend to accumulate subcutaneous fat more readily; their visceral fat increases are comparatively smaller at the same waist size.

Risk Factor 1: Lipid Abnormalities

Either or both:

  • Triglycerides: ≥150 mg/dL
  • HDL cholesterol: <40 mg/dL

HDL ("good cholesterol") removes cholesterol from arterial walls. Low HDL is independently hazardous.

Risk Factor 2: Blood Pressure

Either or both:

  • Systolic pressure: ≥130 mmHg
  • Diastolic pressure: ≥85 mmHg

Note: These thresholds are lower than the general hypertension diagnosis threshold (≥140/90 mmHg).

Risk Factor 3: Blood Sugar

  • Fasting blood glucose: ≥110 mg/dL

Metabolic syndrome is reversible through lifestyle changes.

1. Dietary Improvement

Key changes with strongest evidence:

  • Reduce total calories: Create a 200–500 kcal daily deficit
  • Improve carbohydrate quality: Refined sugars and white rice → whole grains, vegetables, dietary fiber
  • Reduce saturated fat: Fatty meats, butter, fried foods → fish, plant oils
  • Reduce sodium: Target under 6g/day for blood pressure management
  • Increase fiber intake: Vegetables, mushrooms, seaweed improve blood sugar and lipid profiles

2. Physical Activity

Visceral fat responds more readily to exercise than subcutaneous fat.

Recommended exercise:

  • Aerobic exercise (walking, jogging, swimming, cycling): 150+ minutes per week (30 min × 5 days)
  • Resistance training: 2–3 times per week (increases muscle mass, raises resting metabolic rate)

A practical goal: 7,000–8,000 steps of walking daily. "Somewhat out of breath but can still speak" is the target intensity for aerobic fat burning.

3. Quit Smoking / Reduce Alcohol

  • Smoking: Increases visceral fat, lowers HDL. Cessation measurably improves metabolic markers.
  • Alcohol: Raises triglycerides, adds empty calories. If drinking, limit to ≤20g pure alcohol/day.

4. Weight Loss Target

Clinical guidelines target 5–10% body weight reduction over 3–6 months. For an 80kg person, losing 4–8kg typically produces significant improvements across all metabolic markers.
